Here is a nice hammer action play from the Indiana Pacers. The ball is reversed from left to right and thrown to a guard posting up. As the guard makes a move baseline, a hammer screen is set for 3 in the left corner. See More

The play starts out in a floppy set. The 3 comes off the 5 and curls to come to the opposite wing receiving a screen from the 4. The 1 hits the 3 as he comes off the 2nd screen. The 2 cuts off the 5 after the 3 to the wing. Right after the 2 comes off the 5, the 5 turns around and sets the hammer screen on the 2 defender bring him to the corner. Spain often runs a unique entry into their Horns sets. They start with an elevator screen for 3, which gets him the ball in the top of the key area. 1 & 2 then cut to the corners to get into the Horns formation. They have different options depending on who sets the ball screen. If 5 sets the screen, he rolls to the basket and 4 pops. If 4 sets the screen, he immediately goes off of a flare screen from 5.
