Here is a nice hammer action play from the Indiana Pacers. The ball is reversed from left to right and thrown to a guard posting up. As the guard makes a move baseline, a hammer screen is set for 3 in the left corner.

1 passes to 3 then makes a shallow cut to the right wing.
3 passes to 4, 4 passes to 1, and 1 passes to 2 inside.

2 drives baseline and 5 sets a hammer screen for 3.
4 screens for 1 at the top of the key.
