Utah State frequently runs this play to get their leading scorer Sam Merrill (2) the ball coming off a screen, where he can attack the basket or take the open three. The first option looks for their 5 to get the ball at the rim off a double back screen. 2 then receives a stagger screen to get the ball at the top of the key.
On the FMS blog:

1 dribbles to the right side then passes to 5 at the top of the key.
3 cuts to the block.

5 dribble handoffs with 4.
4 passes to 1 on the wing.
2 cuts to the block.

3 & 2 set a double back screen for 5.
1 looks for 5 at the rim.

3 & 4 set a stagger screen for 2.
1 passes to 2 for the shot.
