Once 3 receives the handoff/pitch from 4, 2 clears out the back-side to the left corner and 5 sets a flare screen for 1. 1 comes off the flare screen looking for a shot. If 1 doesn't have a shot, 5 follows and re-screens for 1 with an empty corner side pick-and-roll. 1 attacks off the ball-screen and 5 rolls to the basket. 2, 3 and 4 space out for kickout 3-point shots.
