This is as play I drew up for a group of 6th graders after showing them clips of the Golden State Warriors elevator sets. GSW tends to run them out of horns with some flex cuts and screens but I tried to simplify it for the youngsters. They tried it in a game recently and worked on the second try for a made 3.
Set up in an "inverted horns". Where the strong side guard and shooter cuts to the strong corner. The other guard cuts to the ball for the catch.
Inbound passer cuts through to the opposite corner.
The ball handler dribble attacks to the wing and 3 man cuts through the paint and turns up through the lane through the screen.
