The Boston Celtics ran this ATO action against the Washington Wizards in Game 7 of the second round of the 2017 NBA playoffs.
This is a wrinkle out of their "5" action:

Instead of passing to Isaiah Thomas coming off of the flare screen for a shot, Isaiah cuts through to the opposite side to set a back screen with a down screen option.

2 down screens for 3. 1 sets back screen for 3.
3 has option of either screen.
