Notre Dame used this play in the Elite 8 game against North Carolina. They run a ball movement action that sends the point guard looping into the lane and out to the right wing to set a flare. Then, both posts guys move out to set a ball screen with the first screener slipping.

3 screens down for 1.
2 passes to 5.

1 sets a flare screen for 2.

5 slips to the rim on the ball screen.
4 sets a ball screen behind the slip action.
