3 inbounds to 4, cuts to basket off backscreen from 2, 1 spaces, and 5 flashes to the top of the key  

1 continues to space and occupy his defender, 3 continues to the basket, 4 swings to 5 then screens down for 2 (There is an option for the lob at the rim if 3 point basket is not needed)     

5 reverses back to 2 coming off screen, and 2 hits 3pt jumper.     Game Winner!
