Here is a SLOB that the Portland Trailblazers ran against the Memphis Grizzlies late in fourth quarter of game 2. 2 came off the middle pick from 4 and hit a pull-up jumper.

2 iverson cuts across the top. 4 and 5 screen for 2.

2 uses 4's middle pick. As 2 rubs off 4's outside shoulder, 1 uses 5's flare screen. After 5 sets screen for 1, 5 dives to the rim.
