The Miami Heat run this set a lot with Lebron James playing the 3 position, having the 4 set a down screen for 1 who sprints up to set a ball screen for James. Indiana switched this a lot in the ECF giving James a favorable match up against either Hill or Augustin.
4 sprints down to set a down screen for 1. 4 spaces to arc after screening. 1 sprints up setting 3 a ball screen. 3 attacks whatever side 1 sets a screen on. He can re-screen if 3 doesn't attack right away. 1 pops after screening for a 3. On drive 5 dives to rim.
